Hospice

Hospice care is focused on improving the quality of life for patients with terminal illness. We offer emotional, physical, and spiritual resources to our patients, at home, an assisted living facility, nursing home, or at the hospital. This type of care is specialized for patients who have a life expectancy of six months or less. Although this can be difficult to discuss and come to terms with, our dedicated physicians and staff are here to support you and your family every step of the way.

24 Hour Service

Hospice staff is available 24 hours a day, seven days a week. A nurse will always be on call and can access other team members as deemed necessary.

Bereavement Services

After experiencing the loss of a family member or friend, each person experiences grief differently. Even if you have time to prepare, the loss of a loved one causes many difficult changes in your life. To support you through this process, we provide bereavement services for up to a year after the death of a loved one. Bereavement services include: memorial services, opportunities to discuss feelings or fears, and access community services as needed for family support.

Continuous Care

Continuous care is round-the-clock hospice care provided for brief periods of time when a crisis occurs. The purpose of continuous care is to alleviate uncontrolled symptoms, such as severe pain, bleeding, difficult breathing, nausea, vomiting or diarrhea that won’t stop, agitation or restlessness, or seizures. This form of care honors the person’s request to remain at home (or another facility) rather than being admitted to the hospital.

Coordination of Services

We coordinate and assist patients throughout the entire hospice process, which means we will communicate with your other caregivers to support the objectives outlined in your care plan. Through coordinated care, we are able to address the emotional, physical, and spiritual needs of patients and their families.

General Inpatient Care

When a loved one’s health needs cannot be successfully managed at home or another care facility, they can be admitted to DHR Health to receive inpatient hospice care. During inpatient care, complex symptom management and pain control are provided 24 hours per day. Some people may also need this type of care as they transition from hospitalized curative care to home hospice.

Inpatient Respite Care

When a loved one is diagnosed with a terminal illness, a close family member often assumes the role of caregiver. Over time, the strain of this responsibility can take its toll. To alleviate this burden, we offer respite care. Inpatient respite care allows the terminally-ill loved one to be checked into a care facility for up to five days to allow the caregiver time to rest and reset. During respite care, the patient also benefits by receiving compassionate care that maximizes comfort and well-being.

Interpreter Services

At DHR Health, we understand the diverse needs of our community and we take pride in going above and beyond for every patient and visitor. We offer free interpretation services to help you understand medical language and communicate effectively with your care team. If you require an interpreter, please ask a staff member and one will be provided for you upon request.

Routine Home Care

Hospice services are usually provided in the comfort of your home, but can also be provided at a nursing home or assisted living facility.
